Closures and important dates:

First Day of Lessons: Monday, September 9th, 2019. Last Day: Friday, June 26th, 2020
Closures for Holidays: Thanksgiving (October 14th), Christmas Break (December 23-Friday, January 3rd), Family Day (February 17th), March Break (March 16th-20th, Good Friday (April 10th), Easter Monday (April 13th), Victoria Day (Monday, May 18th)

Liability: by approving this agreement, I agree to hold MATTOMUSIC entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. By approving this form, we agree to waive all our legal rights should the participant be injured in any way, whatsoever.

Make-up Lessons: there are no make-up lessons offered for missed classes for any reason. It is a fair policy to pay our teachers regardless of the student's attendance. Monday/Friday students may receive make-up lessons for classes missed due to stat holidays with exception of Xmas and March break. This is upon client request only and only applicable to the month class will be missed. In lieu of missing classes, we encourage our students to attend the performances.

Cancellation Policy: to cancel the music program, we must receive written notice (email is acceptable) at least 30 days prior to the next scheduled payment date.

I hereby authorize MATTOMUSIC to use and/or publish photographs and/or video including the student's image, likeness and/or voice without compensation.
